Getting your vaccination
You can receive a flu vaccination through the following resources:
- 2024 Flu Outreach schedule
- Flu Ambassador Program in your department
- Occupational Health Services locations starting October 1st
- Your primary care provider. See instructions below*
- Local pharmacy covered by your primary care insurance (i.e. CVS, Costco, Safeway). See instructions below*
- Additional options for UC Employees, Postdoctoral Scholars, and Trainees to obtain flu vaccines. See instructions below*
Submitting your vaccination record
*Please note that when you receive a flu vaccination from an outside provider (such as a local pharmacy or non-UCSF primary care provider), you will need to complete one of the following options to align with University of California policy:
- Starting September 4 use the Digital Screener to attest that you received the current season flu vaccine, with an option to upload the documentation of vaccination to the OHS Portal (VPN MyAccess required from a UCSF computer)
- Upload the documentation of vaccination to the OHS Portal (VPN MyAccess required from a UCSF computer)
- Email the record to [email protected]
- New this year: Any UCSF employee who is also a patient at UCSF Health and gets a flu shot at UCSF Health primary care can consent to share their vaccination record with Occupational Health and no further action is needed by the employee. The record will take 3-4 days to route and upload to Occupational Health.
The flu record should have at least two identifiers: Name, Date of Birth (DOB) or Medical Record Number (MRN). If it only includes one identifier, write your UCSF Employee ID (EID) or DOB on the record.
Declining the flu vaccine
UCSF needs to have a declination record on file. You may submit in the following ways:
- Use the Digital Screener to decline flu vaccine
- Complete the flu declination on the OHS Portal (VPN MyAccess)
- Email the completed Flu Declination Form to [email protected]
Influenza policy information
Influenza is a serious respiratory disease; on average, 34,000 Americans die every year from influenza-related causes. Flu vaccinations are mandatory for employees, physicians and learners across UCSF, including BCH Oakland, per directives from the San Francisco Department of Public Health, the Alameda County Public Health Department, and the UC Executive Order.
For the current influenza season: You must have your flu shot and submit either documentation or submit a declination by November 1, 2024.
