New to UCSF and working in a health care setting? Here's what you need to do before you start.
Requirements for UCSF Health new hires
UCSF employees working in health care settings, including UCSF Health, Zuckerberg San Francisco General Hospital, Laguna Honda Hospital, School of Dentistry, School of Nursing, and UCSF Fresno are required to complete a medical evaluation before the start of their employment. The new hire will:
- Receive an email from Occupational Health Services with an access code and a link to the Access OHS portal
- Complete the required tasks in the Access OHS portal
PLEASE NOTE: Live virus vaccines including but not limited to measles, mumps, rubella, varicella, mpox, and yellow fever, interfere with TB testing. This could cause a 28-day waiting period before TB testing can be completed and the 28-day waiting period can delay your start date. Do not obtain a vaccine outside of OHS without first speaking to us to avoid delays in clearance.

- 1 dose of vaccine formulated for the current season

- Titer showing immunity to all 3 components (Measles, Mumps and Rubella)

Negative TB Symptom Review Questionnaire
AND
Option 1: Negative TB Testing (2-step)
Only one of the two tests may be a TB Blood Assay (QuantiFERON or T-Spot)
- One negative TB test (PPD skin test or TB Blood Assay) within the past 3 months
AND - One negative TB test (PPD skin test or TB Blood Assay) within the past 12 months
Option 2: Positive TB Test History
- Documented positive TB test (PPD skin test or blood test)
AND - Negative chest X-ray within the past calendar year

This is required for those working in patient care areas.
Complete N95 Respirator Clearance Form (SHORT FORM)
- With clearance, RNs are N95 fit tested at the Pre-Employment physical exam
- For all others, an appointment for fit testing is made with EH&S
*For those working in HAZMAT, Certain pharmacy areas, certain hospitality areas, another form is required. LONG FORM
Recommended
COVID-19 vaccination is recommended by UC policy
The policy recommends that covered individuals, including students, faculty and staff follow California Department of Public Health Recommendations for annual COVID-19 Vaccination.
Those who receive the updated annual COVID-19 vaccine may submit their vaccination record via the OHS Portal (VPN / MyAccess Required) or Digital Screener .
UCSF Campus new hires and new Attendings
You may receive an email from your HR representative regarding Occupational Health clearance. Please be prepared to provide the following requirements:
- Seasonal Influenza Vaccine (Only required - Nov 1 through Apr 30)
- Measles, Mumps and Rubella (MMR) immunity
Other vaccine and surveillance may be required based on your role, work exposure and administrative determination. Your HR representative or your supervisor can submit a Service Request for a pre-employment screening. The Service Request is accessible through UCSF computers or VPN only. The new hire will:
- Receive an email from Occupational Health Services with an authorization code and a link to the Access OHS portal
- Complete the required tasks in the Access OHS portal

Graduate Medical Education
Incoming residents, non-ACGME fellows and non-ABMS fellows must complete the pre-placement requirements before coming to UCSF (per UCSF GME policy).
