Immediate Reporting for Serious Workplace Injuries and Violence Incidents

Reporting is required for serious incidents

Creating a safe, respectful, and supportive working environment is a shared responsibility across the UCSF community. Supervisors play a critical role in ensuring employees receive timely support when workplace injuries or violence occur and in helping UCSF meet important safety and regulatory obligations.

If you become aware of any of the following, notify Disability Management immediately:

  • A work-related fatality
  • A work-related injury or illness that results in inpatient hospitalization
  • A workplace violence incident involving an employee, including physical assaults by patients, visitors, or coworkers

California law requires employers to report certain serious workplace injuries, illnesses, and fatalities to Cal/OSHA within strict timeframes. Prompt notification helps ensure UCSF can meet these requirements, coordinate appropriate support for affected employees, and respond effectively to workplace concerns.

Action required

Campus supervisors who are aware of this type of incident should immediately email Judy Rosen, Director of Disability and Leave Administration. If the incident occurs outside of regular business hours, please text or call Judy Rosen at (415) 218-5769. Do not wait until business hours resume the following day.
