Occupational Health Pre-placement Process

Overview

Incoming Health employees must complete this pre-placement process with Occupational Health.

Required documents

In accordance with UCSF Health policy, please bring the following documents to your Occupational Health appointment:

  • Measles, Mumps, Rubella immunizations (x2) or evidence of immunity (titer)
  • TdaP and/or TD immunizations
  • Varicella immunizations (x2) or evidence of immunity (titer)
  • COVID-19 vaccinations — initial series/dose(s) PLUS booster dose(s)
  • Hepatitis B vaccine series or evidence of immunity (titer)
  • PPD, Quantiferon or chest X-ray completed in the last 12 months
    • If you have tested positive for TB in the past, provide record of original PPD showing mm induration and subsequent testing/treatment

If your records are incomplete, Occupational Health will provide you with the means or information to complete them. Some items can be completed at your onsite appointment.

Your Occupational Health visits

You will receive a phone call from Occupational Health Services to schedule your new-hire health screening appointment. On the day of your appointment, please arrive 20 minutes before your appointment time to complete the pre-placement forms. Plan on a one-hour appointment time.

Your first visit

Your initial appointment with Occupational Health may include:

  • Review of medical history and immunization records
  • Review of the physical requirements for your position
  • Brief focused physical examination
  • PPD (TB skin test) placement or symptom review (as indicated)
  • Immunizations (as indicated)
  • Lab tests (as indicated)
  • Chest X-ray (as indicated)

Your second visit

Your follow-up visit may include:

  • PPD (TB test) reading 
  • Review of lab work/titers
