New STAR Award Nomination Forms
Improvements include separate forms for each type of award
As part of workgroup-recommended process changes to the Staff Appreciation and Recognition Program (STAR), new STAR Award nomination forms are now available on the HR website. They have been streamlined to simplify completion and submission.
What’s new
- There are now separate forms for each type of award:
- Checkboxes have been added for ease of indicating area of contribution for SPOT Awards and linkage to Chancellor’s Goals and/or PRIDE values for Achievement Awards.
- Funding information is still required on the form and should be completed prior to submitting for processing.
Transition process
Please begin using the new forms as soon as possible. HR will accept old forms through the end of the calendar year to aid with the transition. However, team achievements must utilize the new Group Achievement Award form.
Users of the Department of Medicine (DOM) award portal can continue to submit using the existing format until the portal is updated.
