Modified Duty Application Now in HR Umbrella

The electronic application is available for Campus managers and supervisors

UCSF Managers or Supervisors who are accommodating their employees with modified or alternate duty due to injury or illness can now do so electronically through HR Umbrella. The application can be accessed under the “Leaves” tab in Umbrella and by selecting “Request Modified or Alternate Duty”.  

Completing the information will create an electronic transitional work plan (TWP) for your employee who is temporarily unable to perform their usual and customary job duties due to a work-related or non-industrial illness or injury. The Transitional Work Plan (TWP) formally documents the temporary accommodations made to allow the employee to return to work safely based on the treating health care provider's temporary restrictions. Agreements made in the plan are based on the interactive process between the employee and the supervisor. 

The application process in HR Umbrella replaces the paper form previously used. The electronic application is available for Campus managers and supervisors, and the application will send reminder emails about the end date of the TWP and/or why the TWP is ending. The application will also allow managers and supervisors to review all TWPs created for their employees.
