The Multi-Location Appointment (MLA) and One-Time Payment (OTP) forms used to process and record all concurrent multi-location hires and/or one-time payments between the UC campuses have been combined.

The new form can be found on UCNet and should be used effective immediately. However, there will be a grace period until June 1 during which UCPath will continue to accept older MLA or OTP forms.

The forms were combined with a goal of streamlining business processes and reducing confusion in processing multi-location transactions. Terminology and instructions on the form have been updated to reflect current business processes and terminology.

Campuses should continue to follow their current business processes for routing the combined form for approvals.
