UC Family Member Eligibility Reverification
Between March and November 2025, some UCSF teammates will be asked to reverify their dependent’s eligibility to be on their UC benefit plans.
To ensure UC’s benefits resources are used wisely, anyone who enrolls new family members in their health and welfare benefit plans must provide documents to verify their family members’ eligibility for coverage. Between March and November of 2025, UC will also reach out to confirm the continuing eligibility of some previously verified family members.
You may receive a packet of verification materials from UnifyHR, if:
- In early 2025, if you enrolled a new family member or members in your UC coverage during last fall’s Open Enrollment.
- At any time throughout the 2025 plan year, if you completed verification more than 3 years ago for the following dependents: spouse, domestic partner, stepchild, grandchild/step-grandchild, legal ward, or overaged disabled child.
You must respond by the deadline shown on the letter from UnifyHR to protect your family members’ coverage.
Help will be available if you have any questions or concerns about the process. UnifyHR is able to provide assistance in multiple languages. If you have any issues obtaining the documents required to verify your dependents’ eligibility, please contact UnifyHR toll-free at 1-844-718-3970.
