Home Health

Involves the provision of home health care for patients who are recovering after a hospital stay, or need additional support to remain safely at home and avoid unnecessary hospitalization. Services may include short-term clinical, rehabilitative, therapeutic, and assistive home health care. This care is provided by clinicians in a wide variety of areas, such as registered nurses, licensed vocational nurses, physical therapists, occupational therapists, speech language pathologists, home health aides, and medical social workers.
