New to UCSF and working in a health care setting? Here's what you need to do before you start.
Requirements for new hires
UCSF employees working in health care settings, including UCSF Medical Center, Zuckerberg San Francisco General Hospital and Langley Porter Psychiatric Hospital, are required to complete a medical evaluation before the start of their employment. Candidates will provide the following:
- Completed Physical Capacity Survey New Hire Packet
- Completed N95 Respirator Questionnaire
- Medical documentation if requesting work restrictions and/or accommodations
Clearance requirements and options for compliance
Option 1
ONE or more of the following options:
- Initial shot or series (one-dose, such as the J&J vaccine) + ONE Booster (initial or bivalent)
- Bivalent Vaccine
AND
- XBB 1.5 Monovalent Vaccine Compliance (ONE of the options below)
- Administration
- Declination
- Attestation
- Deferral

This is required for those working in patient care areas.
Complete N95 Respirator Clearance Form (SHORT FORM)
- With clearance, RNs are N95 fit tested at the Pre-Employment physical exam
- For all others, an appointment for fit testing is made with EH&S
*For those working in HAZMAT, Certain pharmacy areas, certain hospitality areas, another form is required. LONG FORM
UCSF Health hires
You will be contacted by Occupational Health Services by email or phone within 2 business days from Occupational Health Services receiving notice to schedule you for an in person pre-employment screening on-site at Occupational Health Services. You will be prompted to complete the above-mentioned immunization and health screening activities.
UCSF Campus Hires
You may receive an email from your HR representative regarding Occupational Health clearance. Please be prepared to provide the following requirements:
- Proof of COVID-19 vaccinations and booster or exception request*
- Current flu immunizations during flu season
You, your HR representative or your supervisor can request for an in-person pre-employment screening appointment for you. The service request is accessible through UCSF computers or VPN only.
If you are a new attending physician, please call us directly at 415-885-7580 to schedule your appointment.
*Please note UCSF employees working onsite at Zuckerberg San Francisco General Hospital or Laguna Honda Hospital must be vaccinated with the initial series and at least one booster, no exceptions allowed.
Graduate Medical Education
Incoming residents, non-ACGME fellows and non-ABMS fellows must complete the pre-placement requirements before coming to UCSF (per UCSF GME policy).
