An annual performance evaluation and rating for all eligible employees (both career and contract) with appointments of 50 percent or more should be submitted to HR no later than March 15, 2018. Email all completed and fully signed performance evaluations to [email protected].

Supervisors should work with the employee to complete the appropriate performance evaluation form to ensure the completed and signed form is submitted to UCSF HR by the established deadline of March 15, 2018. Annual performance evaluations are based on a calendar year beginning January 1 and ending December 31.
