Voluntary Catastrophic Leave Sharing Program Responsibilities and Procedures

Recipient

  1. An employee will follow established campus and/or unit procedures for requesting approval for a leave of absence due to catastrophic illness, injury or event.
  2. The recipient submits a Leave Request Form, along with medical verification, if required, to the Department Head or designee for approval.
  3. The employee and the Department Head or designee decide how the recipient's request will be communicated in a manner agreeable to the employee.

Recipient's Department Head

It is the responsibility of the head of the Department to ensure that all unit employees are aware of the availability of the Voluntary Catastrophic Leave Sharing Program.

  1. Receives the recipient's Leave Request Form and verifies that the employee meets all eligibility requirements listed in the Eligibility portion of the policy.
  2. Completes the Campus HBS Employee Update Form and sends it to the HBS Processing Center for processing vacation hour adjustments for recipients and donors.

Donor's Department Head

It is the responsibility of the head of the Department to ensure that all unit employees are aware of the availability of the Voluntary Catastrophic Leave Sharing Program.

  1. Receives the donor's Leave Donation Form and verifies that the employee meets all eligibility requirements listed in the Eligibility portion of the policy.
  2. Approves the donor's request.
  3. Ensures confidentiality and anonymity of the donor in order to avoid and semblance of pressure or coercion in the decision to donate vacation leave.
  4. Forwards the Leave Donation Form to the Recipient's Department for processing vacation hour adjustments on the Campus HBS Employee Update Form.

HBS Processing Center

The HBS Processing Center is responsible for processing vacation hour adjustments for donors and recipients.
